Developing the knowledge and technical skills for the effective and safe conduct of common upper extremity and, lower extremity peripheral nerve blocks, trunk blocks neuraxial anesthesia, and associated conscious sedation. Residents will gain knowledge of relevant anatomy, sonoanatomy, ultrasound use procedural skills for both anesthesia and analgesia, as well as knowledge related to the diagnosis and management of complications related to regional anesthesia.
CBD stage for this rotation: Core of Discipline (CoD) Length of this rotation: 1 block PGY Level for this rotation: PGY3, PGY4, PGY5
Other training experiences included in this rotation: ✓ Exposure to and conduct of regional anesthesia procedures according to the clinical range at the hospital site. ✓ Pre-procedure patient counseling for choice of anesthesia and/or analgesia technique and informed consent ✓ Post-procedure patient follow-up, including diagnosis and management of complications of regional anesthesia
In additional to technical skills, knowledge will be gained of the following procedures: ▪ Upper extremity blocks, including: interscalene , supraclavicular , infraclavicular and/or axillary brachial plexus block ▪ Lower extremity blocks, including: femoral nerve block, adductor canal block, sciatic block, popliteal block and/or ankle block ▪ Trunk blocks ▪ Neuraxial anesthesia: spinal, lumbar epidural, ombined spinal-epidural ▪ Advanced: Continuous peripheral nerve blocks
Key objectives for the rotation - by the end of the rotation, the resident should be able to:
Medical Expert
Complete an accurate and complete patient assessment and generate level-appropriate anesthetic and analgesic management plans, including appropriate options for regional or neuraxial anesthesia, demonstrating knowledge of indications, contraindications, and alternatives.
Demonstrate a level-appropriate knowledge of anatomy and sonoanatomy for common upper and lower extremity nerve blocks. Incorporate this knowledge into technical performance of these procedures.
Demonstrate a level-appropriate understanding of ultrasound physics, basic machine function and relevant application to regional anesthesia practice. Demonstrating a technical skill set to permit effective needle visualization and respect for patient tissues and anatomy.
Choose appropriate local anesthetics and adjuncts for the clinical scenario, including knowledge of their pharmacology, benefits and potential complications. Demonstrate effective pharmacologic and non-pharmacological approaches to anxiolysis and procedural sedation.
Demonstrate knowledge of appropriate local anesthetics and adjuncts for regional anesthesia, including knowledge of their pharmacology, benefits and potential complications.
Communicator
Establish an effective therapeutic relationship with patients and effective communication with family. Provide clear and thorough explanations of the perioperative anesthetic plan, in a professional manner. Demonstrate empathy and sensitivity to racial, gender and cultural issues.
Effectively obtain informed consent and disclose the risks,m benefits and alternatives to performing regional anesthesia and analgesia.
Collaborator
Collaborate with surgical teams, colleagues, allied health professionals and colleagues to establish anesthesia plans, including appropriate choice of regional anesthesia technique.
Leader
Manage patient flow and workload in the block room environment with attention to optimal patient care, safety and efficiency.
Professional / Health Advocate
Plan appropriately to address complications and patient safety including procedure briefing, monitoring for complications, and patient follow-up.
