Documentation Of Accidental Dural Puncture

Dear All,

I am collecting the data related to accidental dural puncture on M5, so please follow these steps:

1/ Write down all the relevant information on the white board, including the patient phone number to be reached at after discharge.

2/ Email me the details (MRN, time of puncture, and a very brief words about the management ex. Intrathecal catheter, second epidural,...).

3/ Document under the QI tab, Serious adverse events tab as per the picture:

We have had several accidental dural punctures in the last few months on M5 with incomplete follow up, incomplete documentation in Sunnycare/iPro, or both. Dr. Massouh and I have therefore developed a checklist (see below) that should be completed whenever there is an inadvertent dural puncture. These sheets will be kept near the white board in the M5 OB anesthesia office – please use them to ensure we have appropriate follow up for all accidental dural punctures.

 

Once complete, please tape the checklist to the white board as a visual reminder for follow-up until the patient’s follow-up has been completed.
