Requesting 3rd Party Consults/Lab Results

The following instruction applies to PAC at our Bayview location

Please fill in consult or lab testing requisition (e.g. KMH for cardiology testing/consult), (& release of information form for results),results, give it to the PAA's and let the TL know what is outstanding so that they can follow up on it.
